How can one obtain orthopedic footwear free of charge?
Pursuant to Cabinet of Ministers of Ukraine Resolution No. 321 of 5 April 2012, approving the Procedure for Provision of Technical and Other Means of Rehabilitation (orthopedic footwear), Versavia LLC provides free supply of orthopedic footwear to children with disabilities and to non-disabled children under 18 years of age who have been identified as having disorders in the development of the feet or lower limbs and whose diagnosis has been confirmed by a pediatric orthopedic surgeon or pediatric surgeon.
The following documents are required to obtain technical rehabilitation aids free of charge:
For adults and children with disabilities:
A referral from the Social Protection Authorities (for the manufacture of orthopedic footwear), endorsed with an original wet stamp.
A copy of the medical conclusion from the LKK (Medical Treatment and Control Commission) or the IPR (Individual Rehabilitation Program) indicating that the person requires provision of orthopedic footwear.
A copy of the MSEC certificate, a copy of the passport (for children, a copy of the birth certificate), a copy of the pension certificate, and a copy of the tax identification number (TIN).
For non-disabled children who have not yet reached 18 years of age:
A referral from the social protection authorities for the manufacture of orthopedic footwear, bearing an original wet stamp.
A copy of the medical conclusion from the LKK (Medical Treatment and Control Commission) stating that the individual requires provision of orthopedic footwear.
A copy of the birth certificate. A copy of the tax identification number.
